During a routine patrol, Officer Daniel Carter encounters a bloodied individual staggering down an empty stretch of road. He sends the young man to a nearby rural hospital with a minimal staff, only to discover that both patients and staff are morphing into something horrible. Carter leads the other survivors on a terrible journey into the hospital's basement depths in a desperate attempt to end the nightmare before it is too late.
